Alliance Registration Form

All classes filled on first come, first serve basis, unless affected by a force imbalance.
Individuals affected by a force imbalance will be given priority treatment.
If you are registering online, please do not fax a hard copy from the brochure.
AT&T/Lucent/Avaya bargained for employees and AT&T/Lucent/Avaya term employees are eligible for courses.
AT&T/Lucent/Avaya temporary employees are not eligible.

If you have not received your acknowledgment letter within seven to fourteen (7-14) days after sending in your registration, please call (407) 805-5522 and leave a detailed message.

Please print the Management Payment Voucher (pdf) form out, sign it, and send via mail to an AMPP Coordinator: Karen Aulich (AT&T). Once the form is received, the AMPP Coordinator will verify the information and forward for signature to an Alliance Co-Chair. Original signatures are required on this form.
